CareLogic® Support Programs

Our Specialty Pharmacies have nine Therapeutic Resource Centers with disease focused, specialty trained clinicians. Within those TRCs, our thirteen Specialty Care Management programs, called CareLogic® provide disease-focused clinical interventions by specialized clinicians, including:
- Patient assessments
- Proactive adherence support
- Outcomes tracking
- Patient advocacy and education
- Detailed reporting
- Support for the prescriber-patient relationship and plan of care

Pulmonary Arterial Hypertension (PAH) 

Pulmonary Arterial Hypertension (PAH) is a disease that affects the heart and lungs. PAH causes increased pressure in the pulmonary arteries that carry blood from your heart to your lungs. This increased pressure causes build up of oxygen in your lungs.

PAH affects more women than men and is typically diagnosed between the ages of 20 and 60. Early symptoms of PAH include shortness of breath, extreme fatigue, and chest pain. There is no cure for PAH, but treatment can alleviate symptoms and slow the progression of the disease.

We know that symptoms like fatigue, shortness of breath and chest pain can make living with PAH a daily struggle. The PAH Care Management program provides you with tools and information you need to manage your disease and take your specialty medication correctly. Our CareLogic program for PAH provides you with:

  • 24-hour toll-free access to pharmacists and nurses specially trained in your disease
  • Educational materials to help you manage your condition
  • Coordination with Patient Assistance Programs
  • Access to our social workers
