CareLogic® Support Programs

Our Specialty Pharmacies have nine Therapeutic Resource Centers with disease focused, specialty trained clinicians. Within those TRCs, our thirteen Specialty Care Management programs, called CareLogic® provide disease-focused clinical interventions by specialized clinicians, including:
- Patient assessments
- Proactive adherence support
- Outcomes tracking
- Patient advocacy and education
- Detailed reporting
- Support for the prescriber-patient relationship and plan of care

Asthma

Asthma is a chronic lung condition affecting about 20 million people in the U.S. People with asthma have very sensitive airways and certain triggers cause the airways to become inflamed and swollen. This inflammation can lead to coughing, wheezing and other breathing difficulty. Asthma symptoms can range from a mild nuisance to life-threatening emergencies. There is no cure for asthma, but medication and other treatments can help keep symptoms under control.

CareLogic - Asthma

Our Asthma Care Management program provides you with the tools and information you need to manage your disease and medication treatment. You'll receive regularly scheduled clinical assessments with our trained healthcare professionals. You will also have 24 hour access to nurses and pharmacists who can help you work through symptoms of asthma and help you correctly follow your treatment plan.
